Abstract : Background: Iodine deficiency in endemic goiter areas can cause maternal hypothyroxinemia which during pregnancy can result in impaired psychoneurological development of infants. Objective: This study was undertaken to determine the prevalence of goiter and changes in thyroid hormone levels among pregnant women presenting for prenatal care at the clinics of Shiraz University of Medical Sciences. Methods: Four hundred pregnant women were examined and their goiters were classified according to WHO criteria. Venous blood samples were obtained from 100 of these women and total T3, total and free T4, TSH, T3RU, ? -hCG, and estradiol were measured. Results: Forty women returned and serum hormones were re-checked two or more months after delivery. Fifty three percent of pregnant women had a visible goiter which was nearly the same as the percentage in non-pregnant women residing in the area. The mean, standard deviation, and range of thyroid hormones in this group were as follows: Total T4: 14.7? 2.9إآ g/dl )range: 8.1-21.8(, free T4 14? 2.4 Pmol/l,T3: 189.3? 34.8 ng/dl )range: 122-314(, TSH: 1.37? 0.84إآ g/ml)range: 0.01-5.2(, T3RU: 19.7? 1.6percent)range: 16.7-22.8(. The post delivery levels of T4, T3 and TSH were decreased and T3RU and free T4 increased. The levels of total T4 and T3 in our subjects were comparable to those of iodine sufficient areas such as Japan and Denmark and higher than in iodine deficient areas. Conclusions: It was concluded that the average thyroid function in pregnant women in this region is normal.
